Visit Types
Initial Examination
60-75 minutes and will include a comprehensive examination and treatment intervention provided the same day. Your PT will discuss your plan of care with you and you will be able to schedule additional visits.
Home Safety Assessment
In-home 60-90 minute visit, assessment of full home environment with recommendations for equipment, adaptations and strategies to reduce risk of falls, improve safety and independence with daily tasks and provide resources as needed in order to allow you to remain safely in your home.
Follow up visits
45-60 minutes of treatment intervention tailored to your specific needs and abilities in order to help you meet your goals.
Private pay packages for PT visits
6 visits - save $50
10 visits - save $75
12 visits - save $100
All visits must be pre-paid at the initial visit to receive the package rate. Visits expire if not used within six months of date of purchase.
Medicare Part B
Rates are dependent upon interventions provided, patient's typically will pay 20% out of pocket co-insurance of the contracted medicare rate. This co-insurance may be covered by your supplemental plan if you have one.
Wellness Clients
30-60 minute sessions focusing on strength, conditioning and long-term management of your overall health and well-being. Options for one-on-one sessions and group classes coming soon.
Telehealth Visits
Telehealth visits will receive a 20% discount from the in person visit rate for cash pay clients.
Phone Consultation
Free
Approximately 15 minute phone consultation to ensure that we are able to meet your needs. If not we will be happy to refer you to the appropriate provider.
Power wheelchair evaluation
30-60 minute visit. A PT will assess your abilities and need for a motorized mobility device as well as specific impairements to provide information to medical equipment company. This is often required by your insurance in order for them to pay for your device.
